Forms and medical records
Intake paperwork is emailed to you after your appointment is confirmed, so it is filled out at home rather than in the waiting room. If you would rather complete it on paper when you arrive, tell us when you enroll and we will have it ready.
Before your first visit
Once your appointment is confirmed you will receive: a health history form, a medication list, a consent form, and a practice policies acknowledgment. Complete them before you arrive — the first appointment runs 45 to 60 minutes and it is better spent on your history than on paperwork.
Two things to bring with you or send ahead: a list of every medication and supplement you currently take, including doses, and any prior allergy testing results if you have them. Previous testing is genuinely useful, and it can save repeating work.
Transferring records from another practice
If you have been seen elsewhere for allergy or asthma, ask that practice to send records to us. What is most useful: prior allergy testing, spirometry or lung function results, immunotherapy records if you have been on shots or drops, and clinic notes from any related visits.
Have the other practice fax them to 515-864-0235, or mail them to the clinic. Please do not email medical records: info@allergenix.com is not a secure channel, and we can accept clinical detail there only once you have signed the patient agreement consenting to non-secure communication. Records are not required for your first visit — bring what you have and we will work from it.
Requesting your own records
You have a right to a copy of your medical record. Call 515-644-8448, or fax 515-864-0235, with your name, date of birth, what you need, and where it should go. Requests are usually completed within a few business days, and there is no charge for a copy sent to you or to another treating physician.
Sending records to another physician
Tell us who should receive them and we will send them. This is routine after testing — most patients want their primary care physician to have the results, and we send those as a matter of course when you ask.
